Exercise Program and Training Request
If you would like a personalized exercise program and personal training with an undergraduate intern, please fill out this form.
Maggie personally supervises the interns as they develop and progress your exercise program. She is an ACSM Clinical Exercise Physiologist and has trained many clients throughout her career. Rest assured you are in good hands and are helping the interns gain invaluable experience!
*Please note, personal training is only available when classes are in session. Interns do not work reading days, finals, gaps between semesters, or breaks (spring, autumn, and winter). FSFP is still open during these times, but Kinesiology graduate assistants help with covering these shifts and can provide basic assistance with spotting, form, or exercise/equipment questions.*
Yearly Fitness Evaluation

*If you are a new member, please wait for Maggie to confirm that all paperwork, including the physician clearance, has been received. She will send you the link to schedule your initial fitness evaluation in that confirmation email.*
Pre-Fitness Evaluation Instructions
The fitness evaluation will consist of resting heart rate, resting blood pressure, weight, body composition, cardiorespiratory fitness, muscular strength, muscular endurance, and flexibility.
- Abstain from prior eating and drinking (> 4 hours)
- Abstain from prior strenuous exercise (> 24 hours)
- Abstain from prior caffeine ingestion (> 12-24 hours)
- Abstain from prior nicotine use (> 3 hours)
- Abstain from prior alcohol use (> 24 hours)
- Clothing should permit freedom of movement and include walking or running shoes.
- Participants should continue their medication regimen on their usual schedule so that exercise responses will be consistent with responses expected during exercise training.
- Drink ample fluids over the 24 hour period preceding the exercise test to ensure normal hydration before testing.
Fitness Evaluation Follow-Up Appointment
If you have completed your initial or yearly member fitness evaluation, schedule your follow-up appointment here.
An intern will complete a write-up with your results and basic exercise program recommendations. They will then review this with you and provide a copy at your follow-up appointment. Feel free to use the fitness facility (A022 PAES) in the time between your fitness evaluation and follow-up appointment.

- Lifestyle Spending Account – reimbursement for personal expenses related to health, fitness, family, emotional, and social wellness (your FSFP membership fees are eligible). Up to $125 quarterly for a total of $500 per year. Check out the written instructions below on how to submit a claim.
- Log into Workday.
- Click on “Menu” in the top left-hand corner, then “Benefits and Pay Hub”.
- Under Suggested Links on the left-hand side menu, click on “My FSA/HRA/LSA (HealthEquity)”. This will open another tab in your browser.
- In HealthEquity, click on “Lifestyle Spending Account” on the left-hand side menu.
- Under Lifestyle Spending Account, click on “Submit Receipt or Claim” at the top.
- On the next page, click on “Lifestyle Spending Account Claim – Pay Me Back” square.
- On the next page, click on “Submit Claim Online”.
- Step 1 of 4: Add New Provider, if necessary. Then provide start date and end date is optional. Click next.
- Step 2 of 4: Select Description (for FSFP it would be Fitness Center, Club, Gym, or Studio Membership). Then type in the amount. Select your name under recipient. Click next. Then click “Next – No More Items for Reimbursement” if this is the only item at this time.
- Step 3 of 4: Click “Submit Claim”. Click “OK” on the bottom of the pop-up message.
- Step 4 of 4: Click “Submit Receipt Online Now”. Click “Next”.
- Step 1 of 2: Click on “Add Receipt for Reimbursement”. Choose PDF receipt that Maggie sent you from where you have it saved on your computer. Click “Use This File” on the right-hand side once it uploads. Then click “No More Documentation for Reimbursement”.
- Step 2 of 2: Click “Submit Receipts”. A pop-up message will show confirmation of submission. Click “OK”.
*Make sure to check your preferences for reimbursement under your HealthEquity profile*
- In HealthEquity, click on your initials in the top right-hand corner. Click on “Profile”.
- Click on “Lifestyle Spending Account” on the Modify Profile pop up message.
- Click on “Reimbursement Method” on left-hand side menu.
- Choose either “Direct Deposit” or “Check”. If you choose Direct Deposit, you will need to provide Bank Name, Bank Account Number, Bank Routing Number, and Type of Account.
- Click “Save Changes”.
